If you have bone, joint and muscle conditions (musculoskeletal or MSK for short) you can self refer to the MSK services for physiotherapy.

Self Test Blood Pressure Machine

The self test Blood Pressure machine in the waiting room has been provided to enable patients over the age of 35 years or at the specific request of a Doctor or a Nurse to obtain a reading whilst sitting comfortably and relaxed.

Please take a seat in the waiting rrom for 5 minutes or so before using the Blood Pressure machine. It is important to take this time to relax as this should ensure a more accurate reading.

Once seated at the machine, please follow the easy to use instructions.

Shingles Vaccination Programme

Are you aged 70, 78 or 79 years?

If so, you may be eligible for a vaccination to protect yourself against shingles.

The national shingles immunisation programme for people aged between either 70 on 1st September 2017, 78 & 79 years has been introduced by the Department of Health / Public Health to help protect those deemed most at risk from shingles. This programme  is for a one off lifetime vaccine and will be rolled out to all people between the ages of 70 and 79 over the next few years.

To receive your vaccination against shingles, please book an appointment for one of our designated clinics at reception.

Pneumococcal Vaccinations

Pneumococcal vaccinations are offered to all patients over the age of 65 and  to children and adults with certain long-term health conditions, such as a serious heart or kidney conditions.  If you are at increased risk of a pneumococcal infection you will be given the vaccination just once and, generally, this will protect you for life. However, if your spleen does not work properly or if you have a chronic kidney condition, you may need booster doses  every five years.

Travel Advice

Travel advice can be provided by our nurses, in particular any recommended vaccinations for the countries that you are planning to visit.

Please book an appointment for advice with one of our nurses at least 6 weeks before you travel, as a second appointment will be required with the nurse to actually receive the vaccinations. Travel vaccines have to be ordered as they are not held as stock vaccines. Your second appointment will need to be at least 2 weeks before you travel to allow time for the vaccines to provide full protection.

Some travel vaccines are not available as an NHS service. The nurse will be able to advise you of any applicable fee at your travel advice appointment.

Sickness Certficates

You do not require a doctor's sickness certificate for any illness lasting seven days or less. Your employer may however require you to complete a self-certification form (SC2) which is available from your employer or on the HMRC website.

Evidence that you are sick

If you are sick for more than seven days, your employer can ask you to give them some form of medical evidence to support payment of SSP (statutory sick pay).

It is up to your employer to decide whether you are incapable of work. A medical certificate, now called a 'Statement of Fitness for Work’ (see below) from your doctor is strong evidence that you are sick and would normally be accepted, unless there is evidence to prove otherwise.

You could also provide evidence from someone who is not a medical practitioner, e.g. a dentist. Your employer will decide whether or not this evidence is acceptable. If your employer has any doubts, they may still ask for a medical certificate from your GP.

Statement of Fitness for Work - ’Fit Note'

The 'fit note' was introduced on 6 April 2010. With your employer's support, the note will help you return to work sooner by providing more information about the effects of your illness or injury.

Non NHS Services

Some services provided by general practices fall outside the scope of the NHS and therefore attract charges. Examples include:

• Driving medicals - HGV / PCV / LGV
• Insurance forms
• Private medical reports
• Private medical certificates

The practice nursing team can provide travel advice as an NHS service. However, some recommended travel vaccinations are not available under the NHS. The nurse providing the travel advice will be able to advise you of any fee.

Passport / Photo card Driving Licence Applications do not have to be completed by a doctor, the practice does not therefore provide this service.

The practice has a published schedule of standard fees for services it can provide that are not funded by the NHS. Where the practice is asked to provide a service not covered by the schedule it will confirm the fee for that service before it is provided.

The practice reserves the right to alter fees set out in the schedule. Any such alterations will be notified before the respective service is provided.

If fees are to be paid directly by a third party, their letter of authority containing invoice instructions must be provided at the time of requesting the service.

Without exception, all fees are payable in advance of the service being provided.

Our reception team will be happy to advise you on appointment availability and applicable charges.
